Hospice Care

Hospice at Crown Bay Nursing & Rehabilitation is provided to help care for and improve the quality of life for the patient who has a life-limiting illness with a life expectancy of approximately six months or less. Hospice also provides support for the family of this patient.

Crown Bay Nursing & Rehabilitation offers a specialized hospice program emphasizing a continuum of care. Our program creates an environment that actively involves residents, as well as their physicians and families in choosing their levels of care.

Hospice at Crown Bay includes the following services:

• Inter-team approach to quality care
• 24-hour Hospice nurse and team access
• Counseling and support services for families and caregivers
• Chaplain/spiritual support and counseling
• Respite care
• Bereavement counseling
