Our entire rehabilitation team is committed to creating individualized therapy programs that directly answer those specific needs. In our continuing effort to provide quality care for our residents, Crown Bay Nursing & Rehabilitation has created a multi-disciplinary Wound Care Team that is experienced in treating all stages and types of wounds, including the need for isolation. We will develop an integrated care plan with the physician and continuously assess the need for interventions at all stages of wound healing.
Our sub-acute care is for those residents who are medically fragile and require additional attention. We seek to maximize the quality of life for our sub-acute care population and offer them specialized restorative services. Our goal is to maintain or improve each resident’s highest functional ability.
When someone is suffering from the progressive stages of memory loss, they will need additional support and assistance. Our dedicated team of professionals offers the care needed to help each resident in each unique case.
Hospice at Crown Bay Nursing & Rehabilitation is provided to help care for and improve the quality of life for the patient who has a life-limiting illness with a life expectancy of approximately six months or less. Hospice also provides support for the family of this patient.
A positive and uplifting atmosphere is vital to the success of healing, recovery, and rehabilitation. Crown Bay Nursing & Rehabilitation Center employs outgoing and friendly professional staff to create social programs and activities to fit the residents’ needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and family are always welcome to visit and attend our special events.
Social services provides essential information, manages requests and concerns, coordinates appointments, maintains contact and open communication between the facility, families, and the residents, and helps in care planning and discharge planning for each resident.
